Join this exciting and innovative opportunity to bring therapy services into local pediatric clinics!  The licensed therapist works collaboratively with a clinical and medical team to offer onsite consultation, training, individual therapy, family therapy, and case management services.  Prior experience in a hospital setting or in a medical social work position preferred.  The licensed therapist also works within a large and experienced clinical team at Family Wellness Center.


• Assist in the development and Responsible for implementation of protocols and procedures for collaborative care program

• Serve as Liaison between FWC and pediatrician and support staff

•  Conduct assessments using the Empowered Families Methodology and develop Family Action Plans for families referred by pediatricians • Provide or secure the necessary supports to assist families in the implementation of Family Action Plans

• Provide necessary clinical services to families and/or children referred by pediatricians at pediatric clinics and/or at FWC facilities in a short-term treatment model

• Provide consultation to pediatricians on behavioral health issues related to their patient population

• Attend meetings as needed with pediatricians and administrators

• Collect all necessary client information to facilitate billing for reimbursable services provided

• Ensure that pediatricians receive documentation related to collaborative care services provided to their patients

• Provides mentoring and/or supervision to any staff involved with collaborative care initiative including graduate students and interns, including individual and group supervision

• Attend scheduled FWC staff meetings and training Commented [KM1]: FWC Clinical Service Manager

• Maintains schedule at CPCMG offices and Alvarado offices

• Completes Optum and additional insurance credentialing

• Reviews clinical records in order to ensure documentation compliance that meets or exceeds Medi-Cal and Joint Commission requirements and SDCC standards • Participates in any scheduled audits



  At least four years of direct clinical experience working with youth and adults with mental health needs

  Licensed therapist(LMFT, LCSW, LPCC, or Ph.D.)

  Valid Driver's License

  Strong analytical, communication, and writing skills


 Experience in a hospital or medical-related setting

 Credentialed with insurances

 Experience with electronic health records

 Licensed clinician (MFT, LCSW, or Ph.D.) with two years post-license experience and the ability to supervise licensing hours

 Bilingual in Spanish
