Under the supervision of the Program Coordinator the Connections Coach will provide guidance and life planning support to youth identifying as LGBTQ. Manage participant motivation, removal of barriers, assessments, skill building, including facilitation of relationship wellness groups, and permanency and life mapping.  Assist in program development, operation and team decision making while supporting program outcomes.
  • The development, implementation, and evaluation, as part of a team, of an integrated approach for youth with co-occurring psychiatric and substance disorders and in accordance with quality trauma informed youth programs that adhere to and uphold the standards of South Bay Community Services.
  • Provide quality trauma informed services to youth.
  • Develop Service Plan that’s supports resource navigation in areas relating to independent living skills development, educational and vocational services, health and wellness, crisis intervention and leadership development.
  • Provide assessment, guidance and life planning support to clients during weekly/daily communication.
  • Creating and implementing policies and procedures to ensure safety of staff, youth, and environment.
  • Refer and link participants to appropriate community resources and provide “wraparound” support services.
  • Serve as advocate when navigating resources outside of the agency.
  • Modify support, motivate and provide crisis intervention when necessary. Assist in youth development of self-regulation skills and impulse control.
  • Conduct outreach presentations to increase program utilization and reduce community stigma for LGBTQ and those with mental health needs.
  • Build relationships with community partners through community outreach, participant recruitment and training to referring agencies.
  • Monitor participant progress and maintain accurate records including data entry into approved database.
  • Develop and maintain component services in compliance with funding requirements and ensure quality control of data collection and services.
  • Work independently and as a team member to create innovative ways to meet goals and provide a high level of service that is engaging and meets the needs of the participants.
  • Attend all staff meetings, agency staff development events and appropriate agency-wide committees.
  • Maintain confidentiality and privacy standards of service partner protected health information material in accordance to agency and center policies, and applicable regulations and laws including HIPAA.
  • Establish and maintain linkage relationships with agencies, organizations, civic and community groups.

Master's degree in social work or related field with two years equivalent experience. Two years’ experience working with youth and familiarity with LGBTQ population. Must be willing to develop or possess working knowledge of trauma-informed care and dual diagnosis treatment; experience working in trauma-informed and dual diagnosis programs preferred.  Able to work in a very diverse setting with people of all backgrounds, culture and orientation. Demonstrated leadership and problem solving abilities, training skills, creativity, and public speaking. Ability to provide quality trauma-informed services in a fast-paced environment, specializing in crisis intervention and de-escalation, motivational interviewing, and seeking safety. Ability to (re) prioritize and multi-task is a must. Available to work evenings, weekends and holidays. Able to be cleared to drive and have valid driver’s license with own transportation. COVID-19 vaccination is required.

  • Finger Movement: Picking, pinching, or otherwise working primarily with fingers rather than with the whole hand or arm as in handling
  • Talking: Expressing or exchanging ideas by means of the spoken word. Talking is important for those activities in which workers must impart oral information to clients or to the public, and in those activities in which they must convey detailed or important spoken instructions to other workers accurately, loudly, or quickly.
