POSITION PROFILE:

The Case Manager (Youth and Family Support Worker) is responsible for the development and facilitation of socio-emotional programming for Real Connections and Field Healthy Start school-based resource centers, as well as supporting parent and youth forums. The Case Manager will be experienced in direct client services, family support services, multi-disciplinary case management, community development, and supervision. This position requires independent judgment, creativity, and adaptability.

ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES:

  • Facilitate socio-emotional groups for elementary, middle, and high school students
  • Develop and implement socio-emotional programming for high school students
  • Support the outreach, recruitment, and retention of program participants
  • Provide care coordination and case management services maintaining accurate documentation of client files as well as accurate and timely entries into a client tracking database
  • Develop and maintain positive relationships with students, families, school staff, and community organizations
  • Support parent and youth forums, with an emphasis on the youth leadership component
  • Attend relevant community and school site and district meetings
  • Other tasks as assigned

QUALIFICATIONS (Education, Experience, and Certifications):

  • Bachelor’s Degree in Social Work and/or related Social Science or Associate’s Degree in a related field and 2 years equivalent case management work experience
  • Experience working with diverse populations with varying backgrounds
  • Experience working with school systems is highly desirable
  • Experience in direct client service, particularly experience with youth and families, and community development
  • Experience facilitating groups, such as socio-emotional, youth leadership and peer support.

KNOWLEDGE, SKILLS, AND ABILITIES:

  • Access to reliable transportation with current car insurance
  • Computer literate and proficiency of Microsoft Office package
  • Ability to take initiative, be self-directed, and work independently
  • Ability to manage multiple demands and prioritize when needed
